User Research Acknowledgment

The Alcohol and Gaming Commission of Ontario (“AGCO”) and its vendor, Portage Cybertech, are committed to conducting user research ethically and with care for the personal information provided by selected research participants. This includes the following commitments:

  1. Using your information for the stated Purpose. You agree to take part in a study for the purpose of assisting AGCO in researching, analyzing and/or improving the usability of its web services (the “Purpose”).
  2. Your Participation Is Voluntary. You may take a break or leave the interview at any time without giving a reason. You may also withdraw from the study at any time by contacting the researcher.
  3. We care about protecting your personal information. Any personal information you provided during the study period shall be considered confidential (the “Confidential Information”), and the following parameters shall apply:
  1. the AGCO and its vendor may use such Confidential Information only for the Purpose;
  2. The AGCO and its vendor will use a reasonable degree of care to protect Confidential Information and to prevent any unauthorized use or disclosure of Confidential Information. The AGCO will not disclose your personally identifiable information publicly without your consent. Any personally identifiable data collected by the AGCO during the study shall be used by the AGCO in accordance with this Acknowledgement  and with the Freedom of Information and Protection of Privacy Act, R.S.O. 1990, c. F.31.

By participating in this user research you consent to the following:

  1. To voluntarily be a participant in this User Research and provide feedback;
  2. For the interview session(s) to be recorded and/or transcribed; and
  3. For the AGCO to use the recordings, feedback and notes for the Purpose.

You acknowledge that:

  1. You are 18 years of age or older and
  2. Your participation in the study is on a volunteer, ‘free of charge’ basis.
